Graphnet to provide remote monitoring service across Jersey


Graphnet Health has been selected by Jersey’s Family Nursing and Home Care (FNHC) to provide the island’s first large-scale remote monitoring service.

The number of islanders in Jersey aged 65 and over is set to increase by 30% by 2040, including a 50% increase in the number of people living into their 80s and beyond, according to statistics published in December 2023.

Under the contract, FNHC will use Graphnet’s remote monitoring solution, powered by Luscii, to track the health of frail and vulnerable islanders at home and in community settings.

Rosemarie Finley, chief executive at FNHC, said: “As a community nursing organisation, we see first-hand the impact that frailty, falls, poorly managed wounds and long-term conditions like diabetes can have on our patients and their families.

“This project gives our teams a powerful new way to keep a closer eye on people in their own homes, without adding unnecessary burden to patients or staff.

“By using Graphnet’s remote monitoring technology, supported by the CareTech Challenge funding, we will be able to spot early warning signs, intervene sooner and help more islanders remain independent, safe and well for longer.”

The project is being funded through the Impact Jersey CareTech Challenge programme and delivered by Digital Jersey, with a CareTech grant of almost £800,000, awarded to FNHC to roll out a digital monitoring system for older residents in Jersey.

Using the Graphnet remote monitoring app and connected devices, patients and carers will be able to record key observations at home, such as blood pressure, weight and wound images.

FNHC clinicians will see this information in near real time and receive alerts if readings fall outside agreed parameters, so they can intervene earlier if someone’s health begins to decline.

From January 2026, the project will focus will on frailty and falls for one year, supporting 100 people with remote monitoring at any one time on a rolling caseload.

In the second year of the project, the nursing and care home will Year add pathways for diabetes and wound management, supporting a further 100 people at any one time.

Tom Muller, remote monitoring Lead at Graphnet Health, said: “Jersey has a proud tradition of doing things differently and this project is an inspiring example of a self-governing Crown Dependency using data and digital tools to reimagine how care can be delivered.

“Graphnet Remote Monitoring will help FNHC clinicians focus their time on the people who need them most, while giving patients the reassurance that their health is being monitored every day.

“We are excited to see the benefits this will bring for frailty, falls, wound care and diabetes, and to support plans to scale the service across the island.”

Evidence from similar programmes in England has shown that combining remote monitoring with population health and shared care records can reduce emergency hospital admissions and A&E attendances by up to 50 per cent for some patient cohorts.

Meanwhile, in Graphnet and Luscii announced the launch of the Graphnet RM integrated platform, which combines remote monitoring with population health and shared care records.
