Graphnet and Luscii have partnered to launch an integrated platform which combines remote monitoring with population health and shared care records.
Graphnet Remote Monitoring (Graphnet RM) aims to enable the NHS to identify patients suitable for remote monitoring, deliver proactive out-of-hospital care, evaluate outcomes in real time and access population-level insights.
The partnership is in response to rapid developments in preventative and digitally enabled out-of-hospital care, with forecasts from Graphnet suggesting an additional 500,000 patients will join remote monitoring programmes in the next two years.
Markus Bolton, joint chief executive at Graphnet, said: “Remote monitoring is front and centre of NHS reforms, so consolidating our product sets and customer bases makes perfect sense.
“Luscii has by far the best patient-facing solution we’ve seen, and we’re delighted to incorporate it into our RM platform.”
The first trusts have been enrolled with Graphnet RM becoming available for nurses and doctors to treat their patients remotely through the app and advanced algorithms, with the first go live at Frimley Integrated Care System (ICB).
An evaluation of integrated population health and remote monitoring programmes at Frimley ICB, an early adopter of the approach, found that remotely monitored patients experience up to 50% reductions in hospital admissions and A&E attendances.

Graphnet’s population health and shared care records solution is embedded in 20 integrated care systems, serving 17 million people through population health and shared care record services, and has delivered remote monitoring to more than 75,000 NHS patients and more than 400,000 across Europe.
The Graphnet RM platform builds on this infrastructure, combining it with Luscii’s patient-facing technology to create a fully integrated system.

Luscii’s parent company OMRON Healthcare has sold 350 million blood pressure monitors worldwide and supplied more than two million in the UK in three years.
Meanwhile, since July 2025 healthcare professionals across Greater Manchester have been able to access radiology images directly through the Greater Manchester Care Record, which is powered by Graphnet.
